This form should be used to submit payment requests and/or signed grant agreements to The Miami Foundation. Requests, including supporting documentation, must be received by Friday at 5:00 pm EST for payment within 2 weeks. The Foundation is issuing all payments electronically (ACH/Wire). While we will still allow payments via check, we highly recommend utilizing ACH transfers for faster payouts and enhanced payment tracking. If you have any questions, please contact paymentrequest@miamifoundation.org. The Miami Foundation will nourish our team with “Siesta Week” from June 29 to July 3 to refocus our strength, sustainability, and well-being as we prepare for the months ahead.
