When you first access the application, you will be prompted to respond to eligibility questions. Your response to these questions determines if your system of care meets the requirements of this grant opportunity. After the eligibility questions, you will access the application form. The application form is comprised of nine (9) text responses, four (4) check boxes, and two (2) video uploads. Upload requirements include a project plan, project budget, and letters of commitment from collaboration members, if applicable.. The review team will only access your application and video responses through the submission portal. The application scoring rubric contains further information about what reviewers will look for in each section.. Can an eligible Health Zones project include a for-profit cohort partner?.
