Department of Social Services

About this archived opportunity

Selected grantees will provide the priority population, their infants, and their families in-home case management that links them to community-based agencies and resources for parenting and concrete supports to mitigate risks of substance exposure and maltreatment to infants. Grantees will do so through developing or strengthening collaborative partnerships among health service providers, county and/or tribal child welfare, county public health, behavioral health, and community-based organizations to identify the priority population, take steps to eliminate their substance use and prevent relapse, and ameliorate negative child outcomes associated with parental substance use.
