The Department of Veterans Affairs, Dayton Ohio VAMC, awarded a single source purchase order for emergency boiler refractory repair/replacement IAW 41 U.S.C.1901. See attached Justification for Single Source Award for detailed information. Any responsible source who believes it can meet future requirements may submit a capability statement to Mary Jane Crim via email at maryjane.crim@va.gov. Each response shall include the following Business Information: Contractor Name Address Point of Contact Name, Phone Number, and Email address Contractor SAM UEI Contractor Business Classification (i.e., small business, 8(a), woman owned, hubZone, veteran owned, etc.) as validated in System for Award Management (SAM).
