This is a combined synopsis/solicitation for commercial products prepared in accordance with RFO FAR Part 12. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. 75H71326Q00046 has been issued as a Request for Quotation (RFQ) for a firm-fixed-price commercial products purchase order for Laboratory Supplies on an “As Needed Basis” for the Indian Health Service, Portland Area Office, Western Oregon Service Unit, Salem, Oregon. The requirement includes laboratory supplies for the Western Oregon Service Unit, including the items listed in Attachment 1 - Schedule of Items. Supplies shall be furnished on an as-needed basis, delivered within 10 business days after order, and provided in accordance with Attachment 2 - Specifications Sheet. The anticipated period of performance includes one 12-month base period and four 12-month option periods. The base period is anticipated to be July 9, 2026 through July 8, 2027. Each option period, if exercised, will continue for 12 months following the prior performance period. The actual start date and end date will be stated in the resulting award. The place of delivery/performance is the Western Oregon Service Unit campus, Chemawa Indian Health Center, 3750 Chemawa Road NE, Salem, Oregon 97305. Government acceptance will occur at the Western Oregon Service Unit. Offerors shall submit a completed IHS Indian Economic Enterprise Representation Form with the quotation.
