Notice of Intent to Award / Sole Source Contract Modification The Navajo Area Indian Health Service, Chinle Comprehensive Health Care Facility intends to modify purchase order 75H71024P00883 awarded to Southwest Kidney for Nephrology Clinic services, which expires 04/30/2027. The contractor currently provides clinics on Monday and Thursday from 1:00pm to 5:00pm. The contractor will be adding two more four-hour clinics per week. The Current and Proposed Performance Work Statements for Nephrology services are attached. The Proposed Contractor is the only organization uniquely qualified to provide the required services to ensure that there is no interruption in services that could impact direct patient care.
